چکیده
We would like to thank Drs. Eroglu and Kocyigit for their comment on possible use of colchicine on arteriovenous fistula maturation via neointimal hyperplasia blockage.1 We used simvastatin because it is a pleiotropic molecule capable of modulating the gene expression of vascular endothelial growth factor-A and matrix metalloproteinase-9. In our studies, simvastatin treated vessels had improved vascular remodeling in a experimental murine model of arteriovenous fistula (AVF) with chronic kidney disease using a clinical dose 2. As pointed out by Eroglu and Kocyigit, simvastatin is associated with rhabdomyolysis, hepato-toxicity, and uremia induced cardiomyopathy 3. Taking this into consideration, we have begun to consider local delivery of simvastatin to the vessel wall using nanoparticles or alginate gel wraps to decrease these side effects.

Surgically-created blood conduits used for chronic hemodialysis, including native arteriovenous fistulas (AVFs) and synthetic AV grafts (AVGs), are the lifeline for kidney failure patients. Unfortunately, each has its own limitations: AVFs often fail to mature to become useful for dialysis and AVGs often fail due to stenosis as a result of neointimal hyperplasia, which preferentially forms at t...

BACKGROUND AND OBJECTIVES Arteriovenous fistulas often fail to mature, and nonmaturation has been attributed to postoperative stenosis caused by aggressive neointimal hyperplasia. Preexisting intimal hyperplasia in the native veins of uremic patients may predispose to postoperative arteriovenous fistula stenosis and arteriovenous fistula nonmaturation. DESIGN, SETTING, PARTICIPANTS, & MEASURE...

Creation of an autologous arteriovenous fistula (AVF) for vascular access in haemodialysis is the modality of choice. However neointimal hyperplasia and loss of the luminal compartment result in AVF patency rates of ~60% at 12months. The exact cause of neointimal hyperplasia in the AVF is poorly understood. Vascular trauma has long been associated with hyperplasia. With this in mind in our rabb...
متن کاملDoes Vascular Elasticity Affect Arteriovenous Fistula Maturation?
The vasculopathy of ESRD affects both arteries and veins. The arteries develop arteriosclerosis, which is largely a disease of the media characterized by increased collagen content, calcification, and both hypertrophy and hyperplasia of vascular smooth muscle cells. Veins may exhibit increased width of the intimal and medial layers, and may develop neointimal hyperplasia and calcification. Succ...

Dialysis Access is the “lifeline” for 1.5 million end-stage renal disease (ESRD) patients across the world. Effective hemodialysis treatment requires reliable large caliber vascular access. The preferred hemodialysis access is an arteriovenous fistula (AVF) because of its lower infection rate, lower hospitalization rate and relatively longer longevity compared to grafts.(1, 2) However, the prim...
